Absence And Irregular Attendance Form

Schools have a duty to notify the Local Authority of cases of irregular attendance and continuous absence.
This form should be used to notify the Local Authority of the following authorised or unauthorised absences:
  • 10 days continuous absence (including if the child is likely to be absent for more than 15 days due to illness/injury)
  • Irregular attendance or regular absence where attendance is not improving despite ongoing school intervention. Please do not notify us of all Persistent Absences.
  • Update to any previous notification - this should be done at regular intervals (suitable to that case), or if new information has been received. Please do not just send updates every ten days.
  • Where a Part Time Timetable or Pastoral Support Plan has failed and attendance has not improved or has deteriorated further (previous Part-Time Timetable notifications should have been submitted).
  • Where a child is marked present but is not receiving a suitable education, for instance if a child's anxiety is preventing access to a classroom
Please note, this is a notification only and not a referral.
Upon receiving this form, the Local Authority may contact the school to discuss the situation or suggest further action regarding the pupil’s absence.
Forms will be prioritised on submission date. Schools are expected to continue to provide education, support and safeguarding responsibilities.
The Local Authority expects schools to have met their duty to make reasonable enquiries to establish the whereabouts of any child. Schools are also expected to be aware of and be applying the recent DFE Guidance on working together to improve school attendance. See Attendance Guidance for further information.

By ticking this box, school confirm that they feel they have made all reasonable adjustments, accessed all support available to them and because the child is still unable to access a suitable education this notification could potentially meet the local authorities s19 duties (to make arrangements for the provision of suitable education at school or otherwise than at school for those children of compulsory school age who, by reason of illness, exclusion from school or otherwise, may not for any period receive suitable education unless such arrangements are made for them).
School are expected to provide reasons and attach supporting evidence as to why they feel this notification should be considered by CCAS panel – including (but not limited to) an up to date IHP, EHA, PSP, Medical Evidence, Other Professionals Reports etc. Please do not flag all absence notifications and be aware that use of this tick box does not guarantee this notification will be discussed at CCAS panel (this remains the decision of the LA)
