3 - Client Referral Form

Organisations in Worcestershire can use this form to refer their clients for support from the Adult Learning, Family Learning and Young Adult Learning teams.  Our main hub is the Fairfield Neighbourhood Learning Centre in Worcester, but but we offer outreach learning across the county. This form covers all our activities.

Only use this form if additional information, advice, or guidance is needed to help us understand the support your client requires to access this learning.

To assist us in supporting you and your client:

• Use the "booking on behalf of" feature to provide details for both you and your client
• Use the "special requirements" box to provide additional information about your enquiry, subjects of interest and/or specific needs of the client
• Some parts of our Family Learning programme are not publicly advertised.  For Family Learning referrals, please provide details of the child's school
• Only one registration of interest is needed per family
